Providence Farm Community (“Providence Farm,” “we,” “us” or “our”) respects the privacy of everyone who visits pfcommunity.org. This Privacy Policy explains what information we collect through this website, how we use and protect it, and the choices you have. By using this website you agree to the practices described here.

Scope of this policy

This policy covers information collected through this website and through the calls, texts, emails and forms you send us from it. It does not govern clinical records. Health information created or kept in the course of treatment is protected under the laws described below and our Notice of Privacy Practices, which is given to every client and is available on request.

Information we collect

Information you give us. When you complete a form, call, text or email us, we receive your name, phone number, email address and anything you choose to tell us, which may include information about yourself or a loved one, such as substance use, mental health or insurance details.

Information collected automatically. Like most websites, we collect limited technical information when you visit, such as IP address, browser and device type, pages viewed, time on page and the site that referred you. We use this in aggregate to understand how the site is used.

How we use information

  • To respond to your inquiry and answer your questions
  • To determine whether our program is an appropriate fit and to discuss admission
  • To coordinate with providers or family members you have authorized us to work with
  • To send updates or newsletters you have asked to receive
  • To operate, secure and improve this website

Health information and confidentiality

As a behavioral health and substance use treatment provider, we treat the information you share with us with care. Health information connected to treatment is protected by the Health Insurance Portability and Accountability Act (HIPAA), federal confidentiality rules for substance use disorder records (42 CFR Part 2), and the Illinois Mental Health and Developmental Disabilities Confidentiality Act (740 ILCS 110). We do not disclose that information without your written consent except as those laws allow, such as in a medical emergency or where disclosure is required by law.

Cookies and analytics

We may use cookies and similar tools, such as Google Analytics, to measure site traffic and performance. Any such tools collect technical information and do not receive the content of the forms you submit. We do not use advertising tracking technologies to share health information with advertisers. You can block or delete cookies in your browser settings, and you can opt out of Google Analytics with Google’s browser add-on. Some browsers send “Do Not Track” signals. Because there is no common standard for them, this site does not respond to those signals.

Email and text messages

Email and standard text messages are not fully secure. Please avoid sending detailed clinical information that way. If you text us or agree to receive texts, message and data rates may apply. You can reply STOP at any time to opt out. We do not sell or share mobile numbers or text consent with third parties for marketing.

How we share information

We do not sell, rent or trade personal information. We share it only:

  • With service providers who help us run this website and communicate with you, such as hosting, email and analytics providers, who may use it only to provide those services
  • With providers, family members or others you have authorized in writing
  • When required by law, court order or a valid legal process, or to protect someone from serious and imminent harm
  • In connection with a reorganization or transfer of our organization, subject to this policy

Biometric information

We do not collect biometric identifiers or biometric information, such as fingerprints, face geometry or voiceprints, through this website, as those terms are defined in the Illinois Biometric Information Privacy Act (740 ILCS 14).

How we protect information

We use reasonable administrative, technical and physical safeguards to protect personal information. No method of transmission over the internet is completely secure. If a breach affecting your personal information occurs, we will notify you as required by the Illinois Personal Information Protection Act (815 ILCS 530) and other applicable law.

How long we keep information

We keep inquiry information only as long as needed to respond to you and for our legitimate operational, legal and accreditation purposes. Clinical records are kept as required by Illinois and federal law.

Your choices

You may ask us what personal information we hold about you from this website, ask us to correct or delete it, or ask us to stop contacting you, at any time. Children

Our programs serve adults. This website is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has sent us information, contact us and we will delete it.

Links to other websites

This website links to other sites, including our Community IOP site and the secure third-party processor that handles online donations. Those sites have their own privacy practices, and we are not responsible for them.

Changes to this policy

We may update this policy from time to time. The date at the top of this page shows when it was last revised. Changes take effect when posted.
